Remove Rate Extension command and simplify update notification UI with direct version display and changelog link

- should not suffix translation files `-migrated`
- simplify by not saying 'installed version' and 'your version'. show both version in a single line with [Pasted text] codicon in between
- latest version should be optional
- in footer, below version, also place 'open changelog' link
- remove item 'rate extension'
