fix(studio): show the user's answers to questions and plans in the chat - #20

A chosen option ("You chose: <option>"), a typed answer and a plan
amendment now enter the transcript as user messages, before the reply
they lead to, and are saved with the conversation. Answers do not count
as new turns or guest prompts, and a Chat + Canvas share line stays
above answers given after the share.

When Studio asks a question or proposes a plan, the user's reply now stays in the chat:
Each appears once, before the reply it led to, and is saved with the conversation, so it is there
after a reload and when the conversation is reopened. Approving, rejecting or skipping adds nothing.
An answer belongs to the turn it answers: it does not number a new turn (cards and feedback stay on
their reply) and does not use a guest prompt. A Chat + Canvas share records how many user messages it
held, so its "beyond this point" line stays above answers given after the share.
