Rename the :covariant start to :unconstrained - #100

Both the :unconstrained and :transversal starts are scale-covariant; the name now says what distinguishes this one: it approximates the unconstrained least-squares fit, with no tightness imposed on a transversal. `cover(...; start=:covariant)` and `initialize_cover(...; strategy=:covariant)` still work and emit a deprecation warning naming the replacement.
