feat(stoptb): STOP-601 remarks, STOP-650 other symptoms details, STOP-651 sputum collection date - #401

Store the free-text "Other symptoms" entered when Others = Yes in tb_screening.OtherSymptomsDetails and return it from tbScreening/getAll. …-651) Store sputumCollectionDate (epoch ms) in tb_stoptb_diagnostics.SputumCollectionDate and return it from diagnostics/getAll. The date is cleared when isSputumCollected is false and left unchanged when the field is not sent, so older app versions keep working. OutputResponse.toString() drops null values, so sputumCollectionDate was missing from records with no date. Use toStringWithSerializeNulls() so the key is always present. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

Map the existing tb_confirmed_cases.remarks column. On save, remarks is only updated when the app sends it, so older app builds don't clear it. getAll now returns remarks. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com> (cherry picked from commit ab5a747)
