Fix two release-review findings from #15. Progress seeks now retain chapter-local progression for injected scroll renderers. When prefetch discovers the end of the current chapter, the visible viewport republishes coverage and completion without moving the saved text offset or restarting prefetch.
Both regressions failed before the changes. Validation: 233 reader tests, 2 example tests, reader and example analysis, formatting, and generated-worker consistency checks pass.
