fix(auth): answer concurrent refreshes of one token instead of refusing them - #71
Some apps refresh the same refresh token from several requests at once. Flux Web does this whenever a page loads its data just after the 5-minute ID token expires: each API call refreshes on its own. The provider rotates the token for the first request and answers the others with invalid_grant. Flux turns that into a 401 and shows the login screen, so the person is signed out even though they have a valid refresh token.
The provider's refreshTokenReuseInterval (10s) replay only helps a request that arrives after the first one has finished. Requests that overlap with it lose the rotation update and fail.
Fix
The /api/auth/* route now handles refresh-token grants that present the same refresh token one at a time. Each later request then finds the token just rotated and gets the first response back through the provider's own replay. Every other request is unchanged.
The queue has limits so it can't be used to hold requests back:
Only form bodies with a content-length of 16 KB or less are read. Anything else runs as before.
At most 8 requests per token wait in line; more run as before.
A request waits at most 5 seconds for the one ahead (less than the 10-second replay window), then runs anyway.
The grant type is trimmed the same way the provider trims it.
The queue lives in the process. Production runs one replica; during a rolling deploy two pods can overlap for a moment, which only brings back the old behaviour for that window.
Tests
New test in oidc-token.test.ts: three concurrent refreshes of one token, with refresh_token and refresh_token. Before the fix: [200, 400, 400]. After: all 200, with the same rotated refresh token and a new ID token.
refresh-grant.test.ts covers the order of requests, different tokens running at once, the 5-second wait limit, the 8-request limit, and skipping bodies without a known length.

Walkthrough
The auth route now serializes qualifying refresh-token requests that use the same token. A new test sends three concurrent requests and checks their status codes, shared rotated refresh token, and ID token.

Explanation
The title "fix(auth): answer concurrent refreshes of one token instead of refusing them" matches the main change. The PR adds serializeRefreshGrant, which queues concurrent refresh_token grant requests for the same token per process and wraps auth.handler in src/routes/api/auth/$.ts. The added test in oidc-token.test.ts checks that three concurrent refreshes all return 200 with one shared rotated refresh token. The title is specific and follows the conventional commit style. The title does not mention the process-local scope of the queue, but that detail is secondary.

Send the concurrent refresh requests through the registered route.
The test calls serializeRefreshGrant directly. It does not test the route-to-helper wiring. A regression that removes the helper from /api/auth/* can leave this test passing while concurrent route requests fail. Add a route-level concurrent-refresh test.
