Summary

A session that hits a coding-CLI usage limit is never charged as a crash, and an opt-in mode relaunches it uncharged.

L1: dead-window classification (default behavior, a fix). An operator's own Claude Code StopFailure hook may kill the window, for example to rotate accounts. The session then ended crashed and decide_dev charged it as dev session crashed, which deferred stories once the budget ran out. The adapter now keeps the latched classification at its single exit funnel:

  • After an account StopFailure, the crash is labeled parked.
  • After a usage-limit rate_limit StopFailure (blocked or wait class), or a window death during a pane quota wait, it is labeled usage_limited.

Routing then gives the same uncharged pause as the live exit (crash-relabeled lifecycle crumb). The status stays crashed. A crash whose result.json exists still completes and is never relabeled. An unmatched rate_limit stays a charged crash, as #865 decided.

L2: opt-in uncharged relaunch. New settings: [limits] usage_limit_action = "pause" | "relaunch" (default "pause") and max_usage_relaunches (default 3).

  • Engine: in relaunch mode, a non-completed dev, fix or review session that is usage_limited, or that ended on an account StopFailure, becomes Decision(RETRY, uncharged=True). Persisted refund counters (dev_usage_relaunches / review_usage_relaunches) are subtracted only in budget checks, so attempt / review_cycle and every session id stay byte-identical. At the cap the story falls back to today's uncharged pause. Relaunches are journaled (uncharged, usage_relaunches on dev-decision / fix-decision / review-retry) and notified (dev relaunch: …).
  • Session env: the relaunched session gets BMAD_LOOP_USAGE_RELAUNCH=<n>.
  • Adapter: in relaunch mode, the session ends at once on a wait-class limit and at quota-wait admission while relaunches are left (quota-wait-skipped, reason="usage-relaunch"). An unmatched rate_limit counts as a wait-class limit.

Account rotation stays in the user's wrapper. bmad-loop does not switch accounts. The plugin guide has an "Account rotation" recipe: a wrapper binary, or a pre_session plugin that sets CLAUDE_CONFIG_DIR keyed on BMAD_LOOP_USAGE_RELAUNCH. It lists the caveats: per-directory trust and bypass acceptance, user hooks copied to every directory, and never logging credentials.

Docs: adapter-authoring guide (StopFailure / usage-limit signals), escalations guide (usage-limit pauses), FEATURES, run-state reference, configuration reference (generated).

Test plan

  • tests/test_generic_tmux.py: dead-window relabel for account / blocked / wait / pane quota wait; unmatched stays charged; Stop and transcript activity clear the kind; a result wins over the relabel. Relaunch mode: wait-class exits at once; unmatched exits usage-limited with budget left and at the cap; without the mode the stall timing is unchanged; at the cap the quota wait still admits; with budget left the admission is skipped; unmatched then window death is usage-limited.
  • tests/test_escalation.py, tests/test_engine.py, tests/test_policy.py, tests/test_model.py: uncharged relaunch for dev, review and fix; the cap falls back to the pause; pause mode never relaunches; env probes are skipped for an uncharged decision; the env var is set only after a relaunch; flags go only to primary story sessions; re-arm resets the counters.
  • Zero-token real-tmux E2Es (tests/test_stories_e2e.py): a StopFailure followed by window death pauses uncharged; a relaunch-mode rate_limit StopFailure relaunches the story uncharged with max_dev_attempts = 1 (two sessions, attempt == 2, dev_usage_relaunches == 1, session 2 sees BMAD_LOOP_USAGE_RELAUNCH=1, story done).
  • Ablations: each gating branch was deleted, its test confirmed red, then restored.
  • uv run pytest -q -n logical, uv run pyright, trunk check --all

Closes #959

Summary by CodeRabbit

  • New Features

    • Usage-limit handling can be configured to relaunch eligible dev, fix, and review sessions without charging an attempt or review cycle. Relaunches stop at a configurable per-story limit, defaulting to three; pausing remains the default.
    • Relaunched sessions receive a count indicator in their environment.
  • Bug Fixes

    • Sessions that end unexpectedly after a recognized usage limit, account error, or quota wait can retain that classification and pause without charging an attempt.
  • Documentation

    • Updated configuration and guides with relaunch settings, limits, and account-rotation guidance.

Walkthrough

The pull request adds opt-in, capped usage-limit relaunches for dev, fix, and review sessions. It tracks uncharged relaunches separately from charged attempts and review cycles. The adapter also preserves selected StopFailure and quota-wait classifications when a session window crashes.

Full details: Title check

Explanation

The title clearly identifies the main changes: uncharged handling for usage-limit crashes and opt-in relaunches. The wording is concise and directly related to the changeset, although “never” is broader than the documented behavior for unmatched rate limits.

Full details: Linked Issues check

Explanation

The PR satisfies the coding requirement in #959. The adapter preserves account StopFailure evidence and relabels a later result-less window death as parked. The engine routes the session to an uncharged pause by default. Opt-in relaunch mode also retries eligible incomplete sessions without charging attempts, with a cap and persisted counters. Tests cover account StopFailure, dead-window handling, pause behavior, relaunch behavior, and real-tmux execution.

Full details: Out of Scope Changes check

Explanation

The changes stay within #959 and its usage-limit context. Adapter classification, uncharged routing, optional relaunches, counters, environment signaling, journal records, account-rotation guidance, configuration, and tests support safe account rotation and usage-limit handling. The implementation does not switch accounts. Related dev, fix, and review paths use the same behavior and do not add unrelated functionality.

Full details: Docstring Coverage

Explanation

Docstring coverage is 85.71% which is sufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff with cyclomatic complexity of at least 2. Documentation-only edits and deletions within surviving functions are included; unchanged diff context and fully deleted functions are excluded. Analyzed 21 functions across 11 files. Persist retry intent before saving an uncharged dev relaunch.

usage_relaunch produces Action.RETRY for a non-completed usage-limited session. The engine saves that decision in DEV_VERIFY without the acceptance latch. If an earlier attempt left task.spec_file, _finish_inflight can treat the task as verified dev work and start review after a host loss in this window.

The same save-before-retry window exists for ordinary charged retries in the base revision. Apply the recovery fix to retry intent generally, not only to dev_usage_relaunches. Persist an explicit retry intent, or use a recovery state that cannot enter the DEV_VERIFY plus spec_file acceptance arm, before saving the refund.

Declining in this PR. As the review says, this save-before-RETRY window at DEV_VERIFY already exists in the base revision for every charged retry, and the uncharged relaunch is just another RETRY through the same save. A usage-limited session never reaches verification, so it cannot set spec_file itself; the window needs spec_file left by an earlier verified attempt, which the base has too. The fix belongs in the recovery arms as a whole (persist retry intent, or gate the spec-approval arm on the acceptance latch or a pause record), not in #959. Tracked as a follow-up in the deferred-work ledger.

@pbean Agreed. This is a pre-existing recovery issue, not a defect specific to the uncharged refund. The incomplete usage-limited session skips verification; this failure path requires a retained task.spec_file.

I accept deferring the general recovery fix outside #1021. The concern remains deferred, not fixed. I will not request that refactor in this PR.

Would you like a GitHub issue as well, or is the deferred-work ledger entry sufficient?

P2 Badge Preserve pane usage evidence before skipping the quota wait

In relaunch mode, if a pane-only usage limit is detected and the window dies before stall_exit() checks liveness, this returns an unlabeled crashed result instead of an uncharged relaunch. stall_exit() discards its supplied evidence on the dead-window branch, and _relabel_dead_window() cannot recover it because this branch never opens quota.waiting and there is no StopFailure latch. For a session with measured token usage, the log-tail classifier cannot reliably rescue the classification either, so the engine can charge the crash and exhaust the story's budget despite having just observed its usage limit. Preserve the detected evidence for the dead-window funnel before returning here.

Fixed in cae3e54. stall_exit takes admitted=True from the three quota-wait arms: the relaunch skip, the reset-beyond-cap skip, and the cap. If the liveness re-probe then finds the window dead, the crash is labeled usage-limited from the admitted evidence and crumbed crash-relabeled with kind="stall-exit". That matches how a death inside an open wait is already treated. A bare pane match at the stall gate or timeout still never labels a dead window, because a remain-on-exit pane can show a stale line (the existing #610 rule, pinned by test_dead_window_is_never_usage_limited). New test: test_relaunch_skip_then_window_death_at_the_reprobe_keeps_the_pane_evidence, which fails with the flag removed.

…s at the stall exit (#959)

A relaunch-mode quota-wait skip, a reset-beyond-cap skip, or the wait's
cap exit whose liveness re-probe found the window dead returned an
unlabeled crash, charged, although the admission had just classified
the usage limit. stall_exit now labels that crash usage-limited from
the admitted evidence (crumbed crash-relabeled kind=stall-exit), as an
open wait's death already is. A bare pane match at the stall gate or
the timeout still never labels a dead window. Drop a duplicate _rows
test helper.
