Root cause

The engine writes the deferred-work ledger uncommitted into the unit's working copy (the worktree under isolation, or the project in place). That is deliberate, because finalize_commit later squashes everything above the baseline into one story commit. The writes come from the dev-leg and post-review harvests, the sweep bundle close and reclose, the retry restore after a rollback, and the in-place _defer restore.

The next LLM session runs the BMAD-installed bmad-build-auto. Its step-04 finalization commits the reviewed-diff files and then HALTs blocked with finalization left repository dirty if anything is still uncommitted. The engine turns that into a CRITICAL, and the run pauses on the orchestrator's own dirt. This only happens with a git-visible ledger (tracked, or untracked and not ignored). A gitignored ledger is the default, which is why dogfooding never hit it.

Fix

Right before a review session (every cycle), a repair dev session (fixable retry and _fix_phase), and a dev retry after a non-fixable rollback, and after an in-place defer, the engine commits only the ledger:

  • Exact-path commit with hooks and signing off. verify.commit_paths(..., run_hooks=False) runs add/status/commit under -c core.hooksPath=<empty dir> -c commit.gpgsign=false, so no project hook runs and no pinentry can hang the run. It writes no content of its own: it records bytes the engine already wrote.
  • Squashed by finalize_commit. The pre-session chore(deferred-work): engine bookkeeping before <role> (<key>) commits sit above the baseline and fold into the story commit. The in-place defer's chore(deferred-work): restore ledger after deferring <key> stays on the target branch, like the other ledger chore commits.
  • Silent no-op for a ledger that is gitignored, external, in a shared ticket store, absent, or unchanged. A publication-guard refusal is journaled (engine-ledger-commit-refused). A failure is journaled (engine-ledger-commit-failed) and the session launches as it did before.
  • Sha-recorded, never matched by subject. The persisted StoryTask.bookkeeping_ledger ({baseline, added, shas}) is saved before each commit and invalidates itself once baseline_commit moves. preserve_attempt_commits drops a commit only when the record names it and diff-tree confirms it touches only the ledger. A filter fault degrades to the unfiltered range (attempt-preserve-filter-failed). So a bookkeeping-only range parks no attempt-preserve ref and cannot turn a clean reset into preserve_failed.
  • Untracked-ledger restore. When a bookkeeping commit added the ledger (it was absent at the baseline), reset --hard deletes it. _ledger_is_gits_to_restore now reports that the reset owns the file, so the rollback/defer restore republishes the snapshot instead of losing it.
  • Proof-of-work gate. _harvest_gate_exclude excludes the ledger when a bookkeeping commit added it, so a repair that wrote no code can't pass verification on the engine's ledger bytes.

The control loop still makes no LLM calls, no completion path was added, and the free-text blocking condition is never classified. The fix removes the dirt instead of reinterpreting the HALT.

Tests

  • test_verify.py: hooks-off commit_paths skips .git/hooks and a repo-configured core.hooksPath, and ignores commit.gpgsign with a failing gpg.program; the default still runs hooks; bookkeeping_only_commits filters mixed commits.
  • test_model.py: bookkeeping_ledger round-trip, missing and malformed state, and invalidation on a baseline change.
  • test_engine.py: the helper for tracked, untracked-visible, gitignored, external, unchanged and failing-commit ledgers; case-B rollback restore; the proof-of-work gate; launch spies (story run review ×2, _fix_phase, fixable retry, rollback retry, and no commit on a first-pass dev launch); preserve parks nothing for a bookkeeping-only range and still parks a mixed one; the in-place defer commit, gitignored defer, and the run-end auto-sweep no longer refused as dirty.
  • test_sweep.py: bundle review launches on a clean ledger over two cycles, _fix_phase repair, gitignored and untracked-visible ledgers, and the in-place bundle defer.
  • test_diagnostics.py: routing of the three new journal kinds.
  • E2E test_stories_e2e.py::test_e2e_sweep_review_finalizes_clean_over_engine_ledger_bookkeeping: real tmux, zero tokens. A sweep with review enabled and a fake that HALTs like step-04 on any dirt ends with the bundle done and no escalation.
  • Ablations: removing each opt-in (review, _fix_phase, dev retry), the preserve filter, the _defer call, or the path_clean gate makes its tests fail. With the review opt-in removed, the E2E fails with the exact CRITICAL escalation from review session: … finalization left repository dirty.
  • Full suite: 17976 passed, 116 skipped. pyright reports 0 errors and trunk check is clean.

Note: with rollback_on_failure = false, a review-launch bookkeeping commit makes a defer's manual-rollback pause read "committed work present". That is harmless, and the escalations guide now says which commits are the run's own.

Out of scope

  • A tracked sprint board. The same mechanism could apply, but it hasn't been reported, and the board is deliberately not committed here.
  • Plugin workflow sessions (engine.py:3090). No ledger opt-in.
  • An upstream BMAD request for finalize-exempt paths in bmad-build-auto step-04.

Closes #772

Summary by CodeRabbit

  • New Features
    • Deferred-work ledger updates are now committed automatically before review, repair, and retry sessions, and after in-place deferrals. Commits are skipped when ledgers are absent, unchanged, ignored, or unavailable for publishing.
    • Run diagnostics now record successful, failed, or refused ledger commits. Rollbacks restore ledger changes captured by the engine, and ledger-only commits are excluded from preserved attempt work.
  • Documentation
    • Updated workflow guides to explain ledger commits, rollback and recovery behavior, and related diagnostic records.
    • Recovery instructions now recommend git merge --no-edit for restoring preserved work.

t and others added 5 commits November 1, 2026 11:32
…/gate guards (#772)

Mechanism only; nothing calls the helper yet.

- verify: `commit_paths(..., run_hooks=False)` runs add/status/commit under
  `_hookless_git_config()` (empty core.hooksPath, commit.gpgsign=false);
  `bookkeeping_only_commits` and `path_absent_at_revision` probes.
- model: persisted, self-invalidating `StoryTask.bookkeeping_ledger` read
  through `bookkeeping_for_baseline()`.
- engine: best-effort `_commit_engine_ledger` (record saved before the
  commit); `_ledger_is_gits_to_restore` owns a bookkeeping-added ledger;
`_harvest_gate_exclude` excludes it from proof of work.
- diagnostics: route engine-ledger-commit{,-failed,-refused}.
…r sessions (#772)

Opt the review launch (every cycle), the _fix_phase repair, the fixable
retry and the retry after a non-fixable rollback into the hookless ledger
bookkeeping commit, and drop recorded ledger-only bookkeeping commits from
the attempt-preserve range so they never park a recovery ref.

The ledger bookkeeping commits fix the dirty-worktree failure around review and repair sessions. The only remaining concern is that the run-state reference overstates when a rollback restores the ledger. Operators should be told about the rare capture-failure case. This does not block merging.

Explanation

[#772] The issue requires review to finish done while leaving the pre-existing deferred-work.md edit untouched and uncommitted. The PR instead passes commit_engine_ledger=True for review launches. _commit_engine_ledger commits the ledger with verify.commit_paths(..., run_hooks=False). The added tests validate this commit behavior, not the issue's required unchanged commit state.

Resolution

Preserve the pre-existing ledger edit through review and exempt that edit from the review finalization clean-tree gate. Add a test that asserts the review finishes done and the ledger content and commit state remain unchanged. If the commit-before-review behavior is intentional, obtain and record acceptance of the changed outcome for #772.

P2 Badge Update defer recovery instructions after committing the ledger

For an in-place defer with auto-rollback enabled, preserved attempt work, and ledger changes to restore, this commit creates a new descendant of the reset baseline that is not an ancestor of the already-captured task.preserve_ref. However, _defer_recovery_note() still tells the operator to recover using git merge --ff-only <preserve_ref>, and docs/escalations-guide.md repeats that instruction. Checked local git merge -h: --ff-only means “abort if fast-forward is not possible,” so the advertised recovery now fails even immediately after the defer. Update the recovery command to handle the diverged history, or preserve a topology where the advertised fast-forward remains possible, and test recovery after this ledger commit.

Valid — fixed in 6e5231a. The in-place recovery line is now git -C "<root>" merge --no-edit <preserve_ref>: it still fast-forwards when the branch has not moved, and merges over the defer's ledger restore commit (and any later units' commits) otherwise. New test test_inplace_defer_recovery_line_works_over_the_ledger_commit runs the printed line verbatim after a real plateau defer and checks the attempt work comes back; with --ff-only restored it fails with "Not possible to fast-forward". Docs, model comment and CHANGELOG updated.

…mit (#772)

The in-place defer's ledger restore commit lands on the reset baseline, so
the parked attempt ref no longer fast-forwards and the notice's
`merge --ff-only` aborted right after the defer. Print
`merge --no-edit` instead, which still fast-forwards when nothing moved.

Report the unfiltered range as the parked count: the branch parks at HEAD
and carries the bookkeeping commits the notice's `git log` will show.

Docs: the defer ledger commit is attempted, not guaranteed.

P1 Badge Restore bookkeeping-added ledgers during restart recovery

For an in-place run with rollback_on_failure=True and a ledger that existed untracked at the attempt baseline, this commit makes that pre-existing file tracked. If the run stops or crashes during review, _finish_inflight() takes its restart path and calls _rollback_or_pause(..., restart=True), whose reset deletes the ledger because it is absent from the baseline tree. Unlike the dev-retry and defer paths, restart recovery never invokes either ledger restore helper; baseline_untracked and the artifact-directory cleanup exclusions cannot protect a now-tracked file from that reset. The restarted dev therefore receives no ledger, losing the active backlog's pre-existing rows even if a recovery ref happens to retain a copy. Restore bookkeeping-added ledgers on the restart path before launching replacement work.

Valid — reproduced and fixed in ae6af63. Before the restart rollback, the restart arm now captures a ledger the bookkeeping record marks added (its current text, or the last recorded bookkeeping commit's copy if it is already gone from the tree). After the rollback it republishes that capture through _restore_ledger's compare-and-set: the reset's deletion is the BASELINE anchor, and a rollback that paused without resetting reads the capture back and writes nothing. New test test_restart_rollback_restores_a_ledger_a_bookkeeping_commit_added drives a host death mid-review followed by a restart resume; with the restore call removed, the restarted dev sees no ledger and none survives the run. Two unit tests cover the recorded-sha fallback and the tracked-at-baseline no-op.

…ack (#772)

A ledger untracked at the baseline becomes tracked once the review launch's
bookkeeping commit lands, so a stop or crash during review that resumes into
the restart arm had its reset --hard delete it, operator rows included. The
restart arm now captures that ledger before the rollback (falling back to the
last recorded bookkeeping commit's copy) and republishes it through
_restore_ledger's compare-and-set.

A poll that caught the parent's SessionStart mid-printf dropped it for good
(SignalWatcher consumes a .json name on first sight), so the child's start
became the first one, was admitted as the launched session's, and no
foreign-hook-event-ignored crumb followed: an intermittent py3.14 failure on
PR #1023. Same tmp + rename the child events already use.
