Generated by Claude Code

An entry point declared in a type is written at file scope after it in every
language but C#, and one declared in a namespace is too in C++ and Rust, whose
body then imports the namespace it left. C# gathers an entry point outside a
type into the scope's static holder class, as it does a function.

Fixes #128

Tests the paths the file-level tests did not reach: a type or namespace
handed to the generator directly, which writes its entry point after it,
and an annotated type, whose copy has to carry its annotations.
